How bad are your water works?

This simple form is used by urologists to gauge the severity of urinary symptoms. Its not perfect. but it is the best we have. To use it fill the form out by clicking on the little buttons. You will only be allowed one choice per question. When you have completed all seven questions click the calculate button. This will give you a score out of 35.

If you are coming to clinic or going to see your doctor it would be helpful if you could print the page out after having calculated your score. To do this simply click on the print icon at the top of you browser or choose File, Print from the browser menu.
